New AIM Program at Sutter VNA and Hospice
Most people facing a serious illness would rather be cared for in their own home than in a hospital. Fortunately patients can choose between two home-based programs to realize that goal: Home Care or Hospice. However, each program may limit some patients’ choices. Now, there’s an alternative.
Advanced Illness Management (AIM), a program of Sutter VNA and Hospice, serves as a bridge between Home Care and Hospice. The AIM program provides palliative care and manages the transition from active treatment to end-of-life care. The program is designed for patients with advanced illness who may not yet be terminal or for those patients who are terminally ill but who have not yet chosen hospice.
Some patients eventually opt for Hospice; others improve so much they no longer need Home Care. Either way, people now have more options. For more information about the AIM program (including admission criteria) or to make a referral, call Sutter VNA and Hospice at 800-557-9777.
