www.eyecareamerica.org
The
Seniors EyeCare Program (SEP), formerly known as
National Eye Care Project, ensures that every senior
has access to medical eye care at no out-of-pocket
expense for eligible participants. To qualify,
a senior must be a U.S. citizen or legal resident,
be age 65 or older, not have seen an opthalmologist
in 3 years or more, and may not belong to an HMO
nor have VA benefits. Once admitted to the program,
a referral letter will be mailed within 3 weeks.
Eyeglasses, prescription drugs, hospital services
and fees of other medical professionals are not
included in the service. SEP also promotes annual,
dilated eye exams, raises awareness about age-related
eye disease, including cataracts, and provides
free eye care educational materials. For further
information, please visit the Web site www.eyecareamerica.org or
call the toll-free helpline at 800-222-EYES
(3937) anytime to request free
eye care educational materials or to determine
eligibility for a referral
from one of EyeCare America's 7,500 volunteer
opthalmologists nationwide.

action on your behalf. Additionally, Mission Hospice will be
sponsoring a community forum on health care decision-making
